The frustrating thing about operational problems? They’re usually invisible. Not because they are hidden, but because they’re familiar. They get absorbed into the day-to-day until nobody questions them anymore. Operations tend to break in the same three places: • Handoffs – The movement work moves between people is the moment things fall apart. • Assumptions – When a process lives in someone’s head, it disappears when they’re unavailable. • Informal fixes – The quick patch from six months ago that somehow became standard practice.
