The team agrees it's a temporary solution. The ticket for the proper fix is created, but it's immediately de-prioritized for new feature work. A few sprints later, another developer builds a new abstraction on top of your temporary code, unaware of its fragile foundation. The original context is lost. This is how technical debt metastasizes. The temporary fix wasn't just a static liability; it had a half-life. The longer it sat, the more it decayed, radiating complexity and risk into surrounding modules. What was once a simple surgical fix now requires a major refactoring project that touches multiple services. The most dangerous code isn't the obviously broken part. Three builds. One mistake. Now I have the rule. I kept launching subagents for everything i build because I thought more agents meant better output. It doesn't. My bug-fixing pipeline had three agents. Reproduce, debug, fix. Each handed to the next. Every handoff lost context. The fix agent was guessing. One question fixes this: does the intermediate work matter to you? If you need to see the process, keep it in your main thread. If you just need the result, delegate it. Code reviews work as subagents. The reviewer sees the diff fresh, no memory of how the code was written. Research too. Main thread gets the answer. The 40-file search stays invisible. The architecture never changed. The question did.

We can tell if a software project will survive 3 years by looking at what happened in week one. That's not a flex. It's a pattern. A monolith that should have been modular. Stop calling it 'technical debt' like it's an abstract concept. It's a number. Calculate it. Every shortcut taken during development. Every 'we'll fix it later.' Every integration built with duct tape and hope. It all becomes a line item. It shows up as longer dev cycles for new features. As bugs that keep returning. As the reason your best engineers want to leave. We've walked into codebases where 40% of development time was spent working around decisions made two years ago. That's not building. A lot of developers think production issues come from “bad code.” Most of the time, that’s not true. Production issues usually come from assumptions that were never written down. Assuming APIs respond instantly. Assuming inputs are always valid. Assuming traffic will stay predictable. Assuming services will always be available. None of these are coding problems. They are expectation problems. The real shift in engineering happens when you start building around uncertainty instead of ignoring it. That means: Designing for failure, not just success. Making behavior explicit, not implied. Questioning every “this will always be true” in your system. Code is just the surface layer. The real work is defining what happens when reality disagrees with your assumptions.

Most bugs aren’t caused by bad code. They’re caused by broken assumptions. Somewhere in the system, a developer believed: this value will never be null this service will always respond in time this user flow will follow a predictable path And for a while… it worked. Until reality showed up. A slow network. An unexpected input. A retry that duplicated state. A feature that changed behavior upstream. And suddenly, your “stable” system starts behaving unpredictably. Not because the code is wrong. But because the assumptions were invisible. This is where strong engineering teams stand out. They don’t just write code. They design for uncertainty. They expect partial failures. They make state transitions explicit. They treat external systems as unreliable by default. They log decisions, not just errors. Because robustness doesn’t come from handling known cases perfectly. It comes from surviving unknown cases gracefully. Most Claude Code sessions drift because the plan wasn't tight before the first line ran. I kept building features that were almost right but needed three rounds of correction. The issue wasn't the model — it was me handing it a half-formed problem. There's a Claude Code slash command that fixes this: `/grill-me` Run it before starting any new feature. Claude Code walks you through the plan one question at a time: → It asks targeted questions about scope, edge cases, and architecture decisions → Where the answer is obvious, it suggests one — you just confirm or correct → Where the answer is a real judgment call, it drills until the decision is made The result: by the time you type the first implementation prompt, every ambiguous decision is already resolved. The feature ships on the first session instead of the third. The questions Claude Code asks in /grill-me are the same ones a good senior engineer would ask before starting a ticket. You're just running that conversation before writing any code instead of after.
