Are Compounding Pharmacies Compounding the Problem? One area that sits squarely at the intersection of health and wellness is compounding pharmacies. Few things feel more in the middle. On one hand, compounding pharmacies provide real benefits - customizing medications, filling critical gaps during shortages, and making treatments possible when commercial products fall short. On the other hand, what can get lost in the political fodder are the dangers of non-regulated drug treatments and the very real issues that arise from them. The risks are not hypothetical. In 2012, contaminated steroid injections from the New England Compounding Center led to a fungal meningitis outbreak that killed 64 people and sickened more than 750 (CDC). More recently, Empower Pharmacy has faced recalls and scrutiny for quality lapses - from microbial contamination to using non-pharmaceutical-grade ingredients (Houston Chronicle). Ozempic and other GLP-1 drugs created a frenzy, and when supplies ran short, compounding pharmacies rushed to fill the gap. Patients turned to injectables shipped to their homes, often promoted by wellness clinics and influencers. These products are not FDA-approved and sometimes use untested chemical forms. Still, the practice has normalized self-injection as part of everyday “wellness.” Meanwhile, the current administration is cracking down on pharmaceutical advertising, with off-label and compounded treatments squarely in the crosshairs. A presidential memorandum (Sept 9, 2025) directed HHS and the FDA to tighten standards on direct-to-consumer drug promotions, requiring more prominent risk disclosures and stronger enforcement against misleading claims. At the same time, the FDA is ramping up oversight of compounders and issuing warning letters over misleading marketing (Bloomberg Law). The question isn’t whether compounding labs are good or bad. It’s how we navigate the promise and the peril - and whether we can build a system that balances innovation, accessibility, and safety without losing trust in the process.

Big Pharma just realized they can't buy  their way out of a $240 billion problem. So they're doing something they haven't done in years: actually building drugs themselves. Patent expirations are about to wipe out $240 billion in revenue by 2030 for the top 25 biopharma companies, according to EY's Firepower report. That's not a cliff. That's a crater. For years, Big Pharma had a simple playbook: –Let biotech do the risky early-stage work.  –Step in during late-stage clinical trials.  –Commercialize the drug.  –Print money. It worked brilliantly when there was plenty of innovation to buy on the open market. But that world is gone. So what has changed? Investment money into biotech has dried up. There aren't enough late-stage assets to acquire anymore. So pharma companies are doing something radical: they're bringing R&D back in-house. Over the last decade, R&D spending has jumped from 15% to 33% of revenue at some companies. That's not a tweak. That's a complete strategic shift. Why now? Because pharma realized they have two options: Fund the open market by pouring billions into early-stage biotechs and hope something works. Build their own pipelines and control their own fate. They're choosing option two. What does this mean for the pharmaceutical and biotech industry? The companies that win over the next decade won't be the ones with the best M&A teams. Here are the nuances that go beyond the balance sheet: 🔸 THE 'HIDDEN' P&L: PHARMACEUTICALS In IVF, medication is a massive cost driver. Often, these sales run through separate but related pharmacy entities, meaning the "clinic" revenue understates the business's true scale. Take Innova In Vitro: the clinic reported ₾14M revenue, while its related pharmacy generated ₾9M. To understand the real cash generation of this industry, you have to look at the whole ecosystem, not just the medical entity. 🔸 CASH COWS VS. MARKET GRABBERS Profitability strategies vary wildly. Leadermed is running a fortress balance sheet: a massive 38% EBITDA margin and over ₾11M in cash sitting idle. GGRC is playing a different game: aggressive expansion (65% revenue CAGR since 2020) at the expense of short-term margins (9% EBITDA).
