Most healthcare tech is built from the outside in. Policy first. Models first. Dashboards first. It looks smart and breaks fast. After years inside risk adjustment, RCM, and value based care, the real problem is obvious. Healthcare does not lack intelligence. It lacks shared truth. The same patient shows up differently depending on the system, the report, or the meeting. Teams argue over numbers instead of acting on them. The most valuable thing to build is not another analytics layer. It is an operational truth layer. One patient. One provider. One timeline. Facts you can stand behind when money, audits, and outcomes matter. Not what should be true. Not what a model predicts. What is treated, coded, supported, paid, contestable, and defensible right now. The goal is not automation for its own sake. It is confidence. Confidence to act. Confidence to prioritize. Confidence that the number will not change tomorrow. The best systems do not feel clever. They feel calm. When truth is clear, behavior changes. Providers move earlier. Operators stop reconciling versions of reality. Leaders stop asking why the numbers moved again. Healthcare does not need more tools. It needs one shared reality.

Transforming healthcare with AI requires industry. Industry requires capital investment. Capital investment requires return. Return requires revenue. Revenue requires reimbursement. Medicare (and transitively private insurance) reimbursement policy for clinical AI will define the incentives for the technologies we build and deploy over the next decade. Our existing fee for service codes are what the military would call cost-plus contracting. We pay for every nail, screw, and hour of labor and don't care if that was a good and efficient way to build the tank or not. Our CMS Innovation Center director, Abe Sutton, believes cost-plus is the wrong path for the future of American healthcare. ACCESS and VBC models pay for outcomes. When we have AI that can manage heart failure can we link payment to making people healthier? Doing so would unleash the might of American techno-capitalism on exactly the thing we care about in society, but there are devils in the details.

Really enjoyed chatting with Jason Ross from Privia Health earlier this week and unpacking key strategic points from Privia's Q3 earnings call, including its solid performance in MSSP and the acquisition of Evolent's ACO business. Go to Market Strategy for Product. The core architecture for our EHR is finalized. The system is stable and ready for deployment in a live environment. The logic is defined and the code is objective. We are now shifting focus to the Practice Acquisition Strategy. The immediate objective is not mass scale, but securing the first 5-10 pilot clients to validate the workflow and data integrity.
