Cash flow is king in a DSO. Full stop. EBITDA tells a growth story. Cash flow determines whether you can actually operate, integrate, and scale. A DSO can look profitable on paper and still be cash-constrained: • Heavy upfront equipment purchases • Integration costs that hit before synergies materialize • Working capital stretched by growth or payer mix That’s where capital discipline matters. Operating leases—when used intentionally—are a powerful tool: • Preserve cash for recruiting, marketing, integrations, and patient experience • Smooth cash outlays over an asset’s useful life • Maintain flexibility and keep the balance sheet lighter while supporting clinical excellence Yes, EBITDA matters. It’s the language of buyers, lenders, and partners. But EBITDA doesn’t pay payroll. EBITDA doesn’t fund same-store growth. EBITDA doesn’t save you in a liquidity crunch. Strong DSOs understand the balance: • Preserve cash flow • Optimize EBITDA • Lease where flexibility and capital preservation matter • Buy where long-term ownership and economics win Cash flow buys time. The tRump administration is CUTTING OFF all new enrollments for DME supplies for 6 months. Here is a list of what Durable Medical Equipment (DME) is in moratorium: “As of February 25, 2026, the tRump administration (via CMS and HHS) announced a six-month moratorium on new Medicare enrollment for certain durable medical equipment, prosthetics, orthotics, and supplies (DMEPOS) providers to combat fraudulent billing. While specific, itemized lists of forbidden products are not explicitly detailed in the announcement, the moratorium targets suppliers of equipment commonly prone to fraud, including: 1. Mobility Assistance Equipment: Prosthetics, orthotics, Wheelchairs, power scooters, walkers, canes, and crutches. 2. Respiratory Equipment: Home oxygen equipment, nebulizers, and related accessories. 3. Bedding/Support Surfaces: Hospital beds, pressure mattresses, and lift beds. 4.
