What are the essential components of a validation summary report (VSR) for CSV?

Use a contextdiagram to define the scope of the System of Interest to be validated. A contextdiagram has the System of interest in the center and the interfacing systems, both internal and external surrounding the System of Interest. It might be helpfull to put the most interesting Stakeholders and Processess also in that diagram, or use an extra contextdiagram for Stakeholders and Processess.

In GAMP 5, the Purpose and Scope section defines the validation project's boundaries, objectives, and compliance parameters. It outlines the system to be validated, specifying functions and features in scope, and references relevant regulations and standards. Assumptions and exclusions are documented, ensuring alignment with the Validation Plan. This section sets the project's clear scope, objectives, and compliance framework, aiding in effective project management and regulatory adherence.

This section should describe what we tested during validation, how we tested the system, and provide a list of documents generated during the validation process. We should also mention the internal procedures and standards used for performing the validation. Additionally, it's important to outline the key stakeholders involved in the plan, along with their respective roles and responsibilities. Finally, we should specify the methods or types of testing that were employed.

I don't agree with keeping all the data in VSR. The VSR is like the system's report card. It doesn't need all the technical stuff like detailed test plans, scripts, or lists of problems. Instead, it just mentions that these things exist. The main goal of the VSR is to provide a quick, clear summary of the entire validation process for the GxP system. It should show whether the test scripts passed or failed, without going into every little detail. If there were any issues (deviations) during testing, the VSR should mention them and state their current status. To put it simply, the VSR is like a short summary of the system's report card. The more detailed technical stuff is kept separately for those who want all the specifics.

Validation Results should refer to the references for documents where testing was carried out instead of duplicating the information in the VSR. References for findings should be in the VSR with a brief explanation about the findings instead of explaining it in full as it must already be captured in the CAPA / Incident system.

Major non-compliancies should be mentioned here, together with the risks they implicate for the System of Interest. Preferably a list of risks and suggested or needed measures to prevent the risks from happening. The a validation records should have a connection to the risk- register. The validation records should be reffered to, not integrated in this report. Reason: There might be (too) many validation records to show and this distracts the attention from the purpose of the validation report.

This section should explain whether system can be fully released for GxP use. Conclusion should always mention the exclusions that are applicable to the system. It should also state the frequency for periodic reviews and any recommendations for improvement.

I have found that it's really important to get approval from everyone involved, especially the Quality team, for the report. If there are any issues, things that didn't get finished, or changes from our original plan, we need to write them down in the VSR. Sometimes, when we can't finish everything on time, we make a kind of 'in-progress' report called an Interim VSR to keep things moving

As a minimum, Validation department, QA, and System owner should approve the VSR. Additional stakeholders can be added based on the scope of the project but should not be required. VSR should indicate the restrictions applied to the System in the conclusion section.

This section is not required in the VSR if traceability matrix is maintained. Traceability matrix can be merged into the VSR for small projects.

I would like to add the main sections of the VSR. Title and Document Information - Table of Contents - Executive Summary - A concise overview of key validation outcomes. Introduction - Background, system purpose, and organizational context. Scope and Objectives - Clear validation scope and defined objectives. Validation Approach - Methodology, strategy, and risk assessment explanation. Validation Deliverables- Validation Results - Detailed pass/fail outcomes, deviations, and issue resolution. Any Variations from the plan- Summary of Data and Records - Overview of generated or modified data. Conclusion - Recommendations - Proposed corrective and preventive actions. Signatures and Approvals - References -
