The Eligibility Evaluator is responsible for coordinating, scheduling, conducting, and reporting results of eligibility assessments. This includes both initial and ongoing eligibility determinations. Assessments should thoroughly reflect the current mental health status as well as the needs and risks of youth to properly inform a plan of care.

Essential Duties And Responsibilities

  • Completes the Child and Adolescent Needs and Strengths (CANS); and all other documentation required to assess for program eligibility.
  • Maintains daily communication with Intake Supervisor or designee for coordinating, scheduling, conducting, and reporting results of assessments.
  • Maintains CANS certification and all other necessary certifications, licensures, and qualifications required to conduct assessments.
  • Communicates daily and as needed with Intake Supervisor regarding the assignment, completion, quality, and substance of the assessments.
  • Participates in regularly scheduled supervision for ongoing development and continuous quality improvement.
  • Demonstrates ability to apply feedback for ongoing improvement of all tasks associated with coordinating, scheduling, conducting, and reporting results of assessments and to ensure Choices remains in compliance with all contractual obligations.
  • Conducts work in accordance with established timelines especially with regard to submitting assessment results. Results must be entered into an electronic database and a hard copy provided to the Intake Supervisor and/or designee.
  • Contacts Intake Supervisor or designee immediately should there be a reason to believe a youth will not score eligible for the program.
  • Willingly completes other duties as assigned to advance the mission of Choices.

Qualifications

  • Must have experience providing community-based services to children, youth and their families or caregivers in the areas of children’s behavioral health, child welfare, intellectual and developmental disabilities, juvenile justice, or a related public sector of human services or behavioral health care field for:
    • Three years with a high school diploma or equivalent; or
    • Two years with an associate’s degree or bachelor’s degree; or
    • One year with a master’s degree or higher.
    • Two years of experience partnering with youth and/or families within the context of social services or education preferred.
    • Maintain active CANS Certification.
    • Ability to learn and become proficient with SmartCare database and other required databases.
    • Strong verbal and written communication skills.
    • Highly organized and detail oriented.
    • Access to reliable transportation. If driving an automobile while on company business, must possess a valid driver’s license in state of residence and auto insurance.
    • Demonstrated ability to:


    • Conduct interviews and administer surveys and clinical assessments in person or via telephone to diverse populations.
    • Accurately enter data using Microsoft Excel, Word, online databases, and other related technology.
    • Work effectively with internal and external individuals, including other professionals in the community.
    • Effectively communicate to various internal and external audiences in both person and through various electronic media.
    • Manage time and work effectively with minimal supervision.
$22.60/hour ($47,000/annually)
