I’m headed back from HLTH, reflecting on a number of conversations about advisory boards. Most companies form advisory boards to check a box — not to create value. The best boards are designed with intention: the right people, the right cadence, and a clear purpose. When I build or revive boards for healthcare and digital health companies, the first question I ask is simple: “What decision or direction will this group actually help you make?” Start there, and every other element — recruitment, structure, outcomes — falls into place. Advisory boards aren’t about prestige. They’re about perspective. What needs to change first and foremost is healthcare finance. These insurance bizdiots consume more than half your healthcare dollar, up front. Then we need to make the big hospital systems run efficiently. Finally, we need to stop hemorrhaging 84% of healthcare expenditures on avoidable, behavior-based chronic disease. These three financial problems can be solved with education and automation. 1. Automate the health insurance industry. Provide the EMR, pay 150% of Medicare for procedures documented. 2. Provide an ERP style hospital administration system to find cash leaks and inefficiencies. 3. As the insurance company, offer double digit discounts for improved lab results. Transforming healthcare with AI requires industry. Industry requires capital investment. Capital investment requires return. Return requires revenue. Revenue requires reimbursement. Medicare (and transitively private insurance) reimbursement policy for clinical AI will define the incentives for the technologies we build and deploy over the next decade. Our existing fee for service codes are what the military would call cost-plus contracting. We pay for every nail, screw, and hour of labor and don't care if that was a good and efficient way to build the tank or not. Our CMS Innovation Center director, Abe Sutton, believes cost-plus is the wrong path for the future of American healthcare. ACCESS and VBC models pay for outcomes. When we have AI that can manage heart failure can we link payment to making people healthier? Doing so would unleash the might of American techno-capitalism on exactly the thing we care about in society, but there are devils in the details.

As health systems become more selective, the criteria for adoption are shifting. It is no longer enough to offer innovative technology. Solutions must demonstrate trust, integrate seamlessly into existing workflows, and deliver clear economic value. This shift was evident in discussions at the J.P. Morgan and Digital Health New York Healthcare Investment Trends 2026 seminar, moderated by Chris Lloyd (J.P. Morgan), where the industry’s move away from a growth-at-all-costs mindset toward a more disciplined phase became clear. The market is already reflecting this shift. As David Whelan (Town Hall Ventures) noted, the focus has moved firmly toward unit economics and margins. Yuanling Yuan (SignalFire) added that this environment demands stronger operational discipline, especially as software becomes easier to replicate and differentiation becomes harder to sustain. A critical factor in winning deployments is strong data governance. Trust is built when health systems know that sensitive patient data remains securely within their environment. Companies that design with this principle in mind are gaining faster clinical acceptance. Once trust is established, the focus shifts to outcomes. As Kate O'Donnell (Northwell Holdings) highlighted, health systems are not buying standalone algorithms, but solutions that integrate directly into clinical workflows. In practice, this means most AI-first products will not survive unless they are deeply embedded in real operations. AI, in this context, is not the product. It is a tool to solve real business problems, such as improving cost structures or reducing medical loss ratios. If it cannot deliver measurable impact within existing workflows, it is not creating real value. In other words, the market is shifting from valuing innovation alone to valuing integration and execution. This shift is also reshaping how companies scale. Health systems are increasingly choosing to acquire rather than build new capabilities, accelerating M&A across the sector. Point solutions are being consolidated into broader platforms that deliver more complete workflows. This fundamentally changes how companies should think about scaling. Being acquired is no longer a fallback, but often the most effective path to expand reach and deliver meaningful outcomes. This consolidation trend is also extending beyond the US. Increasing domestic regulatory pressure is likely to push health technology companies to expand into Asian markets to build data scale and strengthen clinical track records. As the industry moves beyond the hype of standalone algorithms and inflated valuations, the focus is shifting to what truly drives adoption: workflow integration and sustainable unit economics. The winners will not be those with the most advanced technology, but those who execute best within real clinical environments.
