Modern healthcare systems work really well at what they were designed to do—codify encounters as snapshots to be consumed by payer networks as claims data. Let's be real: EHRs weren't built for patients or even with patients in mind. That probably wasn't the intent, but that's where we ended up. What was supposed to make medicine more efficient & value-based turned into a hyper-efficient hellscape. Practitioners get 15 minutes with patients to cover two conditions (regardless of whether condition 3 is actually a symptom of condition 1), then send them off to the pharmacy for a scrip that'll probably work. If not, we'll see you in a week to take another swing. & trust slowly erodes while cost of care goes up. Here's the thing: as much as payers say they want to lower costs & practitioners want to cure what ails you, the system can't support the big picture. Efficiency plus quality of care over a lifetime isn't possible when the only participant present for that entire lifetime is the patient—& the patient doesn't have the tools to hold it together. Policy does exist to provide these tools—The 21st Century Cures Act. TEFCA. CMS interoperability mandates. They all provide for an Individual Access Service layer to provide the Patient much needed tooling. The infrastructure exists. But data blocking—not necessarily outright refusal, but slow procedural friction—has strangled the promise. Policy mandates openness. Vendors cite complexity, liability, workflow burden. Policy reenforces. Vendors deliver compliance theater. The loop continues. Meanwhile, the one participant who's present for every encounter, every transition, every gap in care—the patient—doesn't have a seat at the table, just a drafty gown. That gown & a latent paternalism declaring, "You can't handle your own data." "Patients misinterpret labs." "They use a search engine as a second opinion." "They might—gasp!—ask informed questions that take longer than 15 minutes to answer." These concerns aren't entirely wrong. Patients absolutely need help making sense of records. But EHR vendors aren't the right ones to solve it. Providers not only don't have the time to educate, they're being asked to store, reconcile, & make sense of every other provider's data inside their own system. That was never the job. It shouldn't be the job. & it's a burden that doesn't scale. This is exactly the role Individual Access Services was built for. Patients engage a competent agent to represent their interests within the medical records ecosystem. Those records are acquired, aggregated, summarized, & translated into meaningful data for providers & research & for aligning with better healthcare planning at full transparency. This is not disruption for disruption's sake—it is the layer that should exist. The one that finally gives the patient infrastructure to match their stake in the outcome. Make room for the Individual Access Service layer—let go of the burden that was never yours to carry.

Series 1 | Post 3: Are EMRs connected to EHRs? Ravi’s journey continues… When Ravi first visited his neighborhood clinic, all his diabetes visits were stored in the clinic’s EMR. Later, when he shifted to a big hospital, the doctors there used an EHR system. But here’s the catch: the hospital couldn’t see Ravi’s EMR data directly. The two systems weren’t “talking.” Ravi had to carry printouts, repeat tests, and explain his history again. So, are EMRs connected to EHRs? Sometimes yes, but it depends on how they are built. • Some EMRs are standalone or closed islands. • Others are modules inside larger EHR ecosystems (e.g., Epic’s EMR is part of Epic’s EHR). • Many can connect via APIs, HL7, FHIR standards, or Health Information Exchanges (HIEs). Think of it like this: • EMR = CCTV in one building. • EHR = a city-wide surveillance network. • With the right wiring (standards + APIs), the CCTV feed can connect into the city system. For Ravi, that “wiring” would have meant one continuous story instead of fragmented records. Key takeaway: NOT every EMR connects to an EHR. But in modern healthcare, interoperability is the trend, because patients like Ravi shouldn’t fall through the cracks. P.S. Ignore the minor spelling errors in the image. Is Dental Industry Shrinking? Recent Halloween article on decline in cavities and costs by CNBC prompted a deep research opportunity for sikka.ai https://lnkd.in/gTssMVVq Current dynamic reflects a profound structural and economic shift where low-margin, high-volume transactions are being strategically replaced by high-value, specialized procedural volume. ​Raw utilization data derived from thousands of dental practices consistently shows positive growth. Sikka Insights industry trends data, which aggregates de-identified information on procedure volume, indicates year-over-year increases across major service categories. For example, recent analyses confirm positive percentage change trends in Orthodontics (+4%), Restorative (+3%), Diagnostic (+3%), and Prosthodontics (+2%) volume compared to the same period in the prior year.12 This sustained growth is further supported by macro-level forecasts, which project the U.S. dental services market to achieve robust annual growth rates, ranging from a Compound Annual Growth Rate (CAGR) of 4.5% to as high as 12.1% through 2032. According to DeltaDentalinstitute.com, a key finding that explains the paradox of rising revenue amid concerns of shrinking volume is the shift in restorative care patterns. Data indicates an inverse correlation between simple and complex procedures among adults aged 19–64: Direct restorations (simple fillings) volume declined, while Indirect restorations (crowns, bridges, and other complex, high-cost procedures) volume increased significantly. This trend represents quantitative evidence of accumulated deferred care. Patients postponed simple, low-cost fillings (a low-value essential procedure) until the decay progressed to a point requiring a much more expensive, high-acuity intervention, such as a full crown or even an implant. This transformation of pathology from manageable to complex results in a higher average revenue per procedure, creating an artificial inflation of "high-value" volume. ​The cost structure of elective dentistry acts as a primary gatekeeper for volume growth. As cosmetic services frequently fall outside the scope of standard dental insurance, patients are required to cover the full expense out-of-pocket. Given the documented increase in the overall cost of dental care, this out-of-pocket burden represents a significant barrier, especially for simple aesthetic procedures that may be viewed as non-essential. The volume shrinkage observed in this category is a direct function of cost sensitivity. The expert noted the inherent advantages that legacy EHR (Electronic Health Record) companies have as the gate keeper of features. As shown by Epic's (private) recent announcement to develop an internal AI scribe solution, EHR vendors are in a position to limit third-party integration for features such as order mapping, pre-charting or clinical decision support. He noted an increased focus on AI by Oracle Cerner (ORCL, covered by Keith Weiss) post the merger.

Most enterprises don’t operate in a single ecosystem — and they never will. In a multi-system world, vendor neutrality isn’t a nice-to-have - it’s foundational. ERPs, CRMs, procurement platforms, legacy systems, and niche tools all coexist, often for very good reasons. Over time, the enterprise becomes a network of systems, not a stack — and work flows across all of them. That’s why vendor-locked AI struggles in the real world. When AI is tightly coupled to one platform, its usefulness is immediately constrained. It can see part of the picture, act in limited places, and force teams to adapt their workflows to the tool instead of the other way around. Scale becomes difficult, and flexibility disappears. Vendor-neutral AI changes that dynamic. By operating across systems instead of inside just one, agentic solutions can follow the workflow wherever the work actually happens. They can pull context from multiple sources, act within different environments, and adapt as systems change — which they always do. This is something I’ve come to appreciate deeply in the work we’re doing at Oraczen. Neutrality isn’t about avoiding vendors — it’s about respecting the reality of how enterprises actually run. It’s what allows AI to support execution without forcing massive rewrites, migrations, or lock-in decisions upfront.
