The Affidavit of Identity Form is used only in EU as part of the LinkedIn account recovery process. This option is not available to all members and is available only in the EU.

The attached "Affidavit of Identity Form" should be completed and then reply to the email from your LinkedIn Customer Representative with the completed form attached.
