What’s more important, quantity vs quality - or when it comes to our lives, living as long as we can or relishing as much as we can in every moment? If it’s up to me to choose, I’ll take both! Now shift that thought to our patients undergoing mastectomy for breast cancer treatment or prevention. In both cases the primary goal is survival (quantity of life) and while VISUALLY these versions of surgery have undergone massive improvements from radical to nipple sparing mastectomy, loss of SENSATION after mastectomy (impacting QUALITY of life) has not been a big part of the operative discussion or plan. Fortunately, we now are able to address this issue with #nerverepair techniques at the time of mastectomy. This past weekend I had the honor of being program host along with representatives of #Axogen (developer of the Avance nerve graft), a world class faculty, and teams of breast cancer and plastic and reconstructive surgeons from around the country. So much work has occurred before I was ever involved with true pioneers of the technique of nerve repair during mastectomy but the awareness and ability to provide a solution to patients has been an unmet need and these programs are making a huge difference in access to care at the time of surgery when it matters most. My first experience dates back 4 years ago when my colleague #DrAlexMesbahi asked me to travel with him to San Francisco and spend a day with #DrAnnePeled and #DrZivPeled discussing and observing best practices in identifying key nerves that can be used to connect the #Avance nerve graft to the nipple and surrounding skin to provide nerves proper direction for regrowth resulting in better long term return of sensation. Our first case that year and every year since has led to hundreds and thousands of changed lives in the DC area. These programs require a lot of time and expense but the result is priceless - our sisters, wives, mothers, and daughters (and some men of course!) are given not only an opportunity to survive but to feel more normal, maintain their dignity, and to lower their risk of future injuries by avoiding chronic numbness or pain - basically allowing them to thrive as well. It’s always tough missing out on weekend family activities for these programs but I hope my crew at home realizes that it’s for a great cause and if a weekend can result in surgical teams from around the country changing how they operate for the rest of their careers that it’s time well spent!!
